CloseKnit Primary Care is one way we can help you get the care you need, from physical health to mental wellness, nutrition, new parent support, and more, your CloseKnit Care Team is ready to help. Your health goals are important to us, and we’re here for you whenever you need us—including evenings and weekends.

 
How It Works
  1. Open our platform or app and select the “primary care” option
  2. Select who the visit is for (you or your dependent) and identify the type of visit, and if it’s virtual or in-person visit*
  3. Choose the provider and the time and date you’re available
 
What To Expect

We’re here to make care simple and stress-free. Primary care visit types cover a variety of needs and concerns, including:

  • New patient: Support patients using CloseKnit Primary Care for the first time – helps us get to know you and your health history.
  • Wellness/preventive: Address your long-term health goals and wellness, focusing on preventive care with your Primary Care Provider (PCP).
  • Problem/focused: Care from providers who know your history and can identify trends when you're feeling under the weather or have a specific concern.
  • Follow-up: Check in on the status of your health following a previous visit, or to manage health conditions or medications.
 
FAQs

Will I have to choose a Primary Care Provider to use Closeknit?
You do not need to choose a Primary Care Provider (PCP) at CloseKnit to use our clinical services such as behavioral health, urgent care, or nutrition services. If you decide to choose us as your PCP, you will be paired up with a provider licensed in your area and will have full access to a dedicated Care Team, including live chat.

What if I already have a Primary Care Provider?
To access our primary care services, preventive care, and a dedicated Care Team you’ll need to select CloseKnit as your PCP. If you have a PCP already, you can access our other services such as urgent care and behavioral health, and we can keep your PCP informed of the care we provide—if you’d like.

How far ahead can I schedule my primary care appointments?
Some visits, like a sick visit, may be available sooner than an annual preventive visit. Preventive visits are typically scheduled about three months in advance.

What if I need in-person care?
If you’re in need of in-person or specialty care, your Care Team will refer you to a local provider who specializes in the area of need—such as heart conditions, joint care, etc. You may also be referred to see a CloseKnit provider in one of our physical locations, if available in your area.

What if I need lab work?
If lab work is needed before, or following, your visit, your provider will arrange getting an order sent to a facility near you.

When to use primary care vs. urgent care with CloseKnit

If you’re a CloseKnit Primary Care patient, you can see your Care Team for sick, preventive, and follow-up visits. However, there may be times when you want to access urgent care for a quick visit. Follow this guide to know whether you should use CloseKnit Primary Care or CloseKnit Urgent Care.

Am I eligible?

There are only two requirements for registration:

1. Adults 18+ can sign up (dependents ages 2-17 can be added to your account)

2. Must have a CareFirst BlueCross BlueShield health plan, including CareFirst Medicare Advantage and CareFirst Community Health Plan Maryland (some exclusions apply)
